This form is here to provide assistance with owners who require a payment plan to prevents collections activity. Upon receipt the payment plan will be sent to the association for review, modify and/or approval.
The association will confirm any approved payment plan via this form and/or email that must be confirmed by the owner.

1. The association nor AKMPM will follow up on an approved payment plan. It is the responsibility of the owner to make the payments based on the approved submitted payment plan by the association. Any Checks/Money orders) must be received when due
2. Failure to adhere to the payment plan or communicate with the association could lead to automatic placement of your account with the associations law firm
